Some points to bear in mind:

It is customary not to honor a father and son or two brothers with consecutive Aliyot. For example, if we give the Bar Mitzvah boy's older brother the third Aliyah, his father should not have the fourth Aliyah. Another friend or relative should be honored with an intervening Aliyah.

If the honoree is not familiar with the blessings, you may wish to inform him in advance and send this link or print it out and send the text of the blessings in the mail.

Hagba (lifting the Torah) and Gelilah (rolling and covering the Torah) are additional honors. Hagba should be given to someone physically able to lift the Torah scroll and hold it up with three columns visible. Gelilah can be given to someone under Bar Mitzvah, but tall enough to reach the top of the Torah rollers.

An additional honor is opening the Ark and handing the Torah scroll to the Chazan (Cantor or Prayer Leader), and returning the Torah to the Ark after the conclusion of the reading.

Before you choose the individuals you wish to honor with an Aliyah, remember to check with the Gabbai, Shamash or Rabbi (whoever is regularly in charge of determining who gets an Aliyah in your congregation). There are may be individuals who have precedence over the Bar Mitzvah boy and his family, such as a bridegroom or one who has a Yahrtzeit (anniversary of a parent's death). Additional Aliyot can be added if necessary.

Traditionally, the Bar Mitzvah boy is honored with Maftir and reads the Haftara.
